They were flopping around BECAUSE there was loose skin, that's usually the ideal for that. The issue is that she got too big of a "small implant" when she got the lift done and it stretched out the sutures.

She should've just left the big saggy tits, at least those looked good from the front view.

My understanding is that it has to do with the genetics of the collagen in your skin. As in, some people scar more visibly than others.
Someone I know had a lift done and it took about two years for the scarring to fade enough that you have to look for it. But the outline of the nipples is always obvious.

Genetics help, but a good surgeon helps a lot too. I had two stitches about the same size in the same part of the body done by different doctors, one is almost invisible while the other left a pretty big scar.
