I know it's an unpopular opinion but I honestly find slightly chubby Zia hot as fuck, more so than skinny Zia. I get why it's not a popular opinion though and if she gets any heavier then the attraction will drop off a cliff for me.
As far as the job being unhealthy for her she's clearly an absolute mess mentally, and I think you're probably right that this line of work is doing nothing to improve that at best and potentially making it even worse. I do get the feeling she'd have never been able to hold a normal job down though, without cam she'd probably have had a shit life of severe mental illness and poverty.
